Your Health Plan
Your enrolled Healthier SG clinic will create your personalised Health Plan during your first Health Plan consultation at the clinic. During your visit, your doctor will measure your height, weight, blood pressure, etc and enquire about your lifestyle habits (e.g., smoking status). Your doctor will discuss your health screening and vaccination needs, and 1 to 2 health goals or actions that are important for you to take for your long-term health. For those with chronic conditions, your Healthier SG doctor will play a crucial role in managing your health and setting targets for optimal control. This could include weight management, smoking cessation and other specific interventions required to manage chronic conditions.
From 25 Nov 2024, your Health Plan on HealthHub app will offer enhanced features. At a glance, you will be able to view details of your discussion with your enrolled Healthier SG doctor, including shared decisions on actionable goals. Your Health Plan will display diet and exercise recommendations by your doctor with resources available to guide you in your health journey.
For support in building and maintain a healthier lifestyle, the Healthy 365 app enables you to track progress towards your activity goals, access healthy lifestyle activities, receive recommended resources and programmes, and earn rewards for completing challenges.
You can access your Health Plan through your HealthHub app here and use the Healthy 365 app to act on your lifestyle recommendations.
Annual check-in
Your Health Plan is a long-term record of your state of health and steps you could consider taking to maintain or improve your health. It is recommended that you have at least one annual check-in so that you and your doctor are updated on the next steps you should take to maintain or improve your health. The check-in with your clinic may be done in-person or over the phone, e.g., if you have no chronic conditions. For those with one or more chronic conditions, your doctor will recommend two or more visits in the year to better review and manage your condition.
Your enrolled clinic will contact you to schedule your annual check-in.

o Select your preferred Healthier SG Clinic o Book appointment for your 1st Health Plan consultation o Complete the short onboarding questionnaire

During your 1st Health Plan consultation |
Your doctor will:
o Lifestyle adjustments which may include diet, exercise and/or social activities o Regular health screenings, recommended vaccinations, and, for those with chronic conditions, adherence to personalised targets and follow-up plan.
The consultation for your Health Plan and subsequent check-in is free. Prevailing fees for services other than the Health Plan will apply |

Note: 1. If you do not have any chronic conditions, your check-in may be done by in-person or tele-consult with your enrolled doctor. A physical consult must be done at least once every 3 years 2. If you have at least 1 chronic condition*, you need to check-in with your enrolled doctor at least twice annually, with these two check-ins minimally three months apart. One of the check-ins has to be an in-person consultation as advised by your doctor During your check-in, your doctor will:

Charges and Subsidies
Your first Health Plan consult and subsequent annual check-ins to update
your Health Plan are fully subsidised. The nationally recommended screenings
and vaccinations are also fully subsidised. However, if you consult your
doctor for treatment e.g., flu, abdominal pain, and agree to receive these
services, the clinic’s prevailing charges will apply. Investigations performed
(i.e., not part of the nationally recommended screenings and vaccinations)
are also chargeable e.g., blood test to monitor chronic condition.
