Data Sharing for Healthier SG
For Healthier SG to work, healthcare clusters (Singapore Health Services (SingHealth), National Healthcare Group (NHG) and National University Health System (NUHS)), family doctors, agencies and community partners will have to work more closely together to help you with your health needs and support you in leading healthier lives.
To enable strong support for you, clusters and family doctors need to share your health data. For example, your family doctor will collect data from you to create your Health Plan, and to review your progress towards achieving your health goals during check-ins. Some data such as your name, address and contact number may be shared with agencies and community partners (e.g. Health Promotion Board and Agency for Integrated Care) when needed for outreach and support for you. Your cluster, family doctor, agencies and community partners will use the data to assess how they can best help you and may contact you to help with your follow-ups, including sharing suitable or targeted Healthier SG programmes and services that you can participate in to improve your health.
Clusters, family doctors, agencies and community partners will only use your data for specified Healthier SG purposes, with measures in place to safeguard your data. Your identifiable data from Healthy 365 is not currently part of your Health Plan and is not shared. Do consider showing your Healthy 365 application to your family doctor and other partners involved in your care when needed.
Clusters, family doctors, agencies and community partners are committed to protecting your data and maintaining data confidentiality. Data protection, data security and cybersecurity safeguards are in place. Your data is stored securely and accessed by authorised personnel only. MOH also monitors and maintains oversight to ensure your data is properly handled. As part of our commitment, MOH will continue to strengthen our policies, processes and safeguards to ensure proper management and protection of data for Healthier SG.
